TEMP HEALTH PERMIT APPLICATION INFORMATION: Use this link to sign up for your health permit online: https://www.eventeny.com/dashboard/events/event/general/info/?id=15317#:~:text=https%3A//worcesterma.portal.opengov.com/ Once you click on the link, you will need to click on INSPECTIONAL SERVICES, scroll down, and choose TEMPORARY FOOD LICENSE. Each applicant will need to upload their ServSafe Managers and Allergen Awareness certifications as well as a copy of their current Health Dept. permit. There are also 2 City of Worcester documents attached to the application that will need to be filled out. HEALTH & FIRE INFORMATION: If you are going to utilize two or more propane tanks larger than grill size... You will need a propane permit if it is NOT on a food truck (APPLICATION IS IN THE EVENTENY FOLDER) If you are not licensed to Worcester, you will need to get inspected BEFORE the event or make arrangements with the WFD to inspect you onsite. The Health Department will be on-site the morning of doing inspections. There are no refunds if you are sent home by either of these departments. Make sure you have a recent fire inspection along with an updated tagged Fire Extinguisher. You will need to reach out to the Worcester Fire Dept on your own for this inspection and permit.
