Liability Waiver and Photo WaiverI, the Parent/Guardian of the registrant AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS Old Fourth Ward Neighborhood Association, Inc (O4WNA), its affiliates and all of its board members, officers, employees, agents, sponsors, coaches, and team coordinators, and each of them, from any loss, liability, damages or costs they may incur while the registrant is in, upon, or about any facilities or equipment thereon or participating in any program affiliated with O4WNA.
