O4W Youth Soccer Program

Liability Waiver and Photo WaiverI, the Parent/Guardian of the registrant AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS Old Fourth Ward Neighborhood Association, Inc (O4WNA), its affiliates and all of its board members, officers, employees, agents, sponsors, coaches, and team coordinators, and each of them, from any loss, liability, damages or costs they may incur while the registrant is in, upon, or about any facilities or equipment thereon or participating in any program affiliated with O4WNA. I, the Parent/Guardian of the registrant agree and acknowledge that participation in O4WNA programs may involve inherent danger and risk, including, without limitation, the risk of physical illness or injury, death or property damage. I HEREBY ASSUME FULL RESPONSIBILITY FOR, AND RISK OF ILLNESS, BODILY INJURY, DEATH OR PROPERTY DAMAGE to myself or such registrant while in, about or upon any facilities or equipment thereon or participating in any program affiliated with O4WNA. As a member of O4WNA, the registrant and I agree to act in such a fashion as to represent O4WNA, the sport and its players and staff in a sportsmanlike manner. I grant permission to O4WNA to use any photographs, motion pictures, recordings or any other record of the registrant's participation in the Program for any legitimate purpose including but not limited to promoting, advertising and marketing purposes. Any and all photographs, motion pictures, recordings or other records of registrant's participation in the Program are and will become the sole property of O4WNA.
