The City of Buford Form will need to be completed along with any required docs requested and stated in RED in the City of Buford Health form. Please note, all your docs and forms will need to be submitted within 30 days of your approval. If not, we will go to the next food vendor to approve. There are no refunds |

City of Buford Health form must be completed by 30 days of your acceptance. If not submitted, your space will go to next food vendor. There will be no refunds. |
