By completing this form, you acknowledge that you understand and accept the duties and responsibilities outlined in this form, and commit to full participation in your capacity as a volunteer Admin/Director/Assistant Director of Anime Weekend Atlanta for the 2020 Convention Planning Cycle, January 1 through December 31 of the aforementioned year.
You understand that attendance at all Director and Crew Meetings is expected for volunteers at your level. Real life always comes first, but please let a managing director know about absences (ahead of time) or if you need to be gone for an extended period. You understand that this is a volunteer position and there will be no monetary compensation for any services rendered. You agree to hold AWA, LLC harmless for any and all injuries as well as accept any risks that may accompany volunteering with AWA, LLC.

Anime Weekend Atlanta Volunteer Waiver and Release of Liability
I understand and agree to the terms and conditions outlined below. I further agree to indemnify and hold harmless AWA, LLC (AWA), its affiliates, directors, and/or associates from any and all claims which are brought by, or on behalf of, myself and which are in any way connected with my participation in the convention. In being permitted to participate in any activities organized, operated, or sanctioned by AWA, I hereby acknowledge and agree to the following terms and conditions:
1. I understand that participation in the convention is of a voluntary nature. I understand and agree that in return for volunteering, I will receive a special behind the scenes access to the convention that otherwise would not be possible. There are special perks associated with being a volunteer that may change, but no monetary compensation will be given in return for my services.
2. I have read, understand, and agree to abide by the AWA Crew Code of Conduct, the general AWA Code of Conduct, Dress Code, Prop & Weapons Policy, and other AWA policies and procedures set forth in the AWA Crew Handbook, Program Guide, and official website (www.awa-con.com).
3. I understand and agree that the AWA policies and procedures, and terms and conditions of participation set out in the crew guidelines are effective and apply to me at all times when present on and around the Convention Center and Official Hotel(s) during AWA, as well as any other space or venue affiliated with AWA, LLC at any time.
4. I acknowledge and agree that acceptance of any badge, membership, or other form of admission issued by AWA constitutes acceptance of agreement to the following:
4a.) AWA is released from any liability for loss or damage to persons or property, infringement of any right, or any claim, or cause of action of any kind.
4b.) AWA and its designees are authorized and permitted to use my name, voice, and/or likeness and all reproduction thereof in all media now and hereafter known for all purposes in perpetuity.
5. I understand that the materials and tools provided, such as radios, hand-trucks, etc., by AWA, LLC are and remain the property of AWA, LLC and I agree to return these tools and any remaining materials to Anime Weekend Atlanta at the end of my volunteer service. Should I fail to return any property belonging to AWA, LLC, I understand and agree that I am held liable and will submit, replace, or otherwise compensate AWA, LLC upon request within 30 days.
6. I further understand that participation as a Volunteer may involve physical activities and/or requirements (i.e., standing for long periods of time, lifting objects weighing 15 pounds or more, dancing, role playing) and may be inherently dangerous, carrying the risk of serious or grievous injuries, including bodily injury, damage to personal property, and/or death, which will demand the exercise of reasonable care to avoid said dangers. I am voluntarily participating in this activity with knowledge of the hazards and potential dangers involved, and agree to accept any and all risks of personal injury and property damage. I further acknowledge that the activities will be conducted in facilities that may or may not be open to the public.
7. Should it be necessary for me to receive medical care while participating as an AWA Volunteer, I give the AWA personnel permission to use reasonable discretion in obtaining medical care for myself, and grant permission to any physician or other qualified medical providers, including but not limited to nurses and ambulance personnel to render whatever medical care they deem necessary and that all charges for services, equipment, and supplies incurred for such treatment shall be my sole responsibility, and I agree to pay for all that was associated with rendering medical care to me.
8. I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY. BY SUBMITTING MY VOLUNTEER FORM, I AGREE TO IT OF MY OWN FREE WILL.
