WAIVER, RELEASE, & ASSUMPTION OF RISK
Thank you for your willingness to volunteer for the City of Woodburn. The City of Woodburn (the “City”) is committed to conducting its programs, services, and activities in a safe manner and holds the safety of City volunteers in high regard. However, volunteers and parents/guardians of minor volunteers must recognize that there is an inherent risk of injury when choosing to volunteer. You are solely responsible for determining if you or your minor child/ward are physically fit and/or adequately skilled to perform the volunteer activities.
READ CAREFULLY BEFORE SIGNING
By signing and submitting the volunteer application you recognize and acknowledge that there are certain risks of physical injury to volunteers providing and/or engaging in volunteer activities for the City, I voluntarily agree to assume the full risk of any and all injuries, death, damages, or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said volunteer activities. I further agree to waive and release all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of or related to the volunteer activities, against the City, its officials, officers, employees, agents, and/or other volunteers, collectively or individually.
I further declare that I am aware of the activity contemplated and the hazards connected therewith; understand that I may be a passenger in vehicles operated by City employees; and understand that I will be a guest and not a passenger for hire or other consideration. I further authorize the person in charge to secure any necessary emergency medical services in the event that such are necessary and I am unable to make conscious and competent decision as to my need thereof. I further agree to pay for such services and to save the City and its employees harmless therefrom.

Gracias por su dispocision de ser voluntario para la Ciudad de Woodburn. La Ciudad de Woodburn (la “Ciudad”) se compromete a conducir sus programas, servicios y actividades de manera segura y mantiene la seguridad de los voluntaries de la Ciudad en alta consideracion. Sin embargo, los voluntaries y padres/guardian de voluntaries menores de edad deben reconocer que hay riesgos inherentes de lesiones cuando optan ser voluntaries. Usted es unicamente responsable en determinar si usted o su niño/pupilo es fisicamente adecuado y/o adecuadamente habil de realizar las actividades de voluntario.
LEA CUIDADOSAMENTE ANTES DE FIRMAR
Entiendo y reconosco que los voluntarios proporcionando y/o participando en actividades de voluntarios para la Ciudad tienen ciertos riesgos de lesiones fisicas. Y estoy de acuerdo voluntariamente de assumir todos los riesgos posibles y toda lesion, muerte,daños o perdidas, independientemente de la gravedad, que mi niño/pupilo de menor de edad o yo puedamos sufrir en resultado de las actividades voluntarias. Tambien estoy de acuerdo en renunciar y divulgar a todas las reclamasiones que yo o mi niño/pupilo de menor de edad tengamos (o proceder a mí oa mi hijo/pupilo) en resultado de o relasionado a las actividades voluntarias, encontra de la Ciudad, sus funcionarios, oficiales, empleados, agentes y/o otros voluntaries, colectiva o individualmente.
Yo declaro que estoy consiente de la actividad contemplada y los peligros entre su conexion; entiendo que pueda que sea un pasajero en un vehiculo operado por empleados de la Ciudad; y entiendo que sere un invitado y no pasajero empleado o alguna otra consideracion. Asimismo, autorizo a la persona encargada de asegurar los servicios médicos de emergencia necesarias en caso de que tales fuesen necesarios y fuera yo incapaz de tomar la decisión consciente y competente en cuanto a mis necesidades. Estoy de acuerdo en pagar por tales servicios y para salvar a la ciudad y sus empleados inofensivos de los mismos.
