About the application
This application is for food trucks / tents that want to provide on-site food and/or non-alcoholic drinks (potentially beer, wine, and/or seltzers) at this year's West End Arts Festival.
About the event
Terms & Conditions
- Vendors are responsible for charging/paying tax on all food items
- Food tents/trucks will be at festival during all show hours. No exceptions will be made
- Vendors must comply with all health code ordinances and laws
- Food tents/trucks will be inspected by the local health officer Saturday and Sunday prior to 10am
- All tables must be fully covered with linens or coverings that extend to the ground on all sides. Booths should be kept clean, tidy, and professionally presented at all times.
- All cars will be parked in designated areas only (POLICE ORDER)
- We will be applying for a social district permit, allowing alcohol to be freely transported by guests to through the festival grounds. We cannot guarantee that will be allowed however
- If you intend on selling beer and wine during the event, please note that this will require certain insurance requirements

- If we receive permission to sell alcohol at the event, please choose the most appropriate response.
- If you will be selling beer, wine, and alcohol - you/your business will be responsible for properly checking identification. Are you able to do this?

- I understand that if my business does not hold a La Grange Village Food Permit, I will need to pay an additional $25 temporary food permit fee. The LGBA will turn this fee over to the Village.
