We invite you to display your quilt or fiber art item(s) at the Havana Quilt & Fiber Art Show and Sale.  Whether it's a treasured piece passed down through generations, a creation that tells a meaningful story, a creation that you made or purchased, we would love to have you submit a request to have it/them displayed at our event.  You must submit an application for each item.  Items will be displayed at the 1908 Event Center in the middle of Havana.  Registration is required and closes October 15, 2025.  There is no fee to apply.  

PLEASE READ THIS INFORMATION CAREFULLY AND THOROUGHLY

Havana Main Street (HMS) will take extreme measures to protect your item.  However, since the items will be displayed in public, no guarantees can be made that are not within our control.  By signing this entry form, you acknowledge that you understand and will not hold HMS responsible for any damage done to your item which is out of our control.  

SUBMIT AN APPLICATION FOR EACH ITEM YOU WISH TO DISPLAY

1.      Design must be safe and suitable for a family atmosphere.  

2.      No political, tobacco, alcohol or drug themes allowed.  

3.      Items must be delivered to the Havana Welcome Center during these times (unless other arrangements are made in writing or email):

• W,T,F, Oct. 22nd–24th, 1:00–5:00
• Sat., Oct. 25th, 10:00–3:00   
• T,W,F, Oct. 28th – 30th, 1:00–5:00

4.      When you drop your item off you must present the Entry Form and speak with a HMS volunteer in person for inspection to note any issues with the item and receive a PICKUP SLIP to present when you pick your item up. 

5.      If you are supplying a special write-up to be attached or support items, etc. for your item, they must be labeled with your name and listed on the Entry Form in the “Comments” section.  If your item has multiple pieces, is delivered in a bag or box, make sure each piece, box and bag are clearly labeled with your name and phone no.

6.      Items must be picked up:

• Sat., Nov. 1st, 4:00 – 6:00 from the 1908 Event Center at 102 E 7th Ave.
• Sun., Nov. 2nd, 1:00 – 5:00 from the Slappey Building at 101 W 7th Ave.

You will be required to provide the PICKUP SLIP provided to you when item is dropped off.  Items not picked up by 5:00, Nov. 2nd will become the property of HMS unless written or email approval for a later pickup date is provided from HMS.  After this time, the items will be located at the Welcome Center, Slappey Building, 101 W 7th Ave, Havana, FL.

7.      I understand that photos taken of my item may be shared with publications, posted on social media and used for future promotional purposes.  I consent for the release of photographs of my item without further written permission.

8.      I understand that the public will be allowed to take pictures of items.

9.      I understand that HMS will take precautions to protect my item, however, I understand that insurance is my own responsibility.

10.    I understand that by completing questions 22, 23, and 24, I can list my item for sale. I acknowledge that Havana Main Street will not be involved in the transaction and that I am responsible for responding to all inquiries from potential buyers. I also understand that Havana Main Street is not liable for the sale and that I must personally handle the delivery of the item to the purchaser.
