About the application

Welcome to the Sherwood Music Fest at Stella Olsen Memorial Park! This is an electronic vendor agreement and by completing this application, you are validating and approving this agreement electronically. 
 
Please fill out this application completely, missing sections will require us to decline the application. You will have to provide a credit/debit card on the application, however you will not be charged unless accepted as a vendor.  If you would prefer to pay via check, you will receive an email upon acceptance and will have seven days to send in a check. If you have any questions, contact Florian at (503) 530-6909.
 
Please review our Vendor Rules and Regulations before submitting your application.
 
Previous FRP vendors receive a discount when charge is completed.

Terms & Conditions

Upon being accepted, we will send you a contract which you will need to return by May 31, 2024 as well as General Liability insurance documentation in the amount of $1,000,000 per occurrence/ $2,000,000 aggregate (minimum) naming Florian Raqueño Productions and City of Sherwood, its officers, agents, employees and volunteers as additional insured.  The business name on your insurance MUST match the business name listed on this application. 
 
No refunds are permitted after acceptance.
