About the application

Welcome to the 2024 Taste of Mableton! Browse through our Food Booth application and let us know if you have any questions. Maxine Wilson at drmaxinewilson@gmail.com will be your primary contact for the event. 
 
The Food Booth Application is reserved for restaurants to sell and share prepared food, drinks, and snack items. 
All applications are processed on a first-come, first-serve basis. Please make sure to sign, date and upload all agreements. Also, make sure to upload pictures of all required food and safety options. 
 
Applications will not be approved until all documentation has been provided and verified. Selection of Food vendors will be based upon those who provide complete applications first.

  • Please fill out, sign, date and upload the "Self-inspection Checklist"
  • I will adhere to the requirements in the Cobb County "Conditional Employee or Food Employee Reporting Agreement"
  • Please upload a copy of your Safe Serve Certification
  • Please upload a picture of your food storage, temperature control, hand washing & utensil washing equipment (whichever are applicalbe)
  • Please describe what on-site handwashing/sanitation, utensil washing, food thermometer, temperature food control you will use at the event.
