- Membership(s) for Minors are purchased through this form.
- If applicable, the badge will be mailed to the address of the Attending Adult - otherwise, it can be picked up with Attending Adult with government ID at KNC Registration.
Minor Membership(s) amount will be charged after we have verified the Attending Adult's confirmation code.
- Minor (13-17): $65 Early Bird Price until June 30th, 11:59pm PST ---> Standard Membership $80
- Youth (5-12): $40 Membership
- Baby (4 and Under): Free Membership
This is an agreement between KuroNeko Cultural Association and you, the parent or legal guardian of a minor attending KuroNekoCon 2024.

Terms & Conditions
- Attendees who are under the age of 18 must submit this signed permission slip.
- Attendees age 13 to 18 must have the attending adult listed on this form present in the venue at all times.
- Attendees under the age of 13 must be physically accompanied by the attending adult listed on this form at all times.
- If the attending adult is the same person as the legal guardian, enter the legal guardian's name in both sections of the form. The attending adult must be 18 years of age or older and be a registered attendee.
By signing this permission slip you affirm that:
- You have the legal authority to enter into this agreement on behalf of your minor child.

You give permission for your child to attend KuroNekoCon 2024.

You agree that KuroNeko Cultural Association staff, organizers, and agents have no responsibility whatsoever to monitor or supervise the minor at any time during KuroNekoCon, and agree to not hold KuroNeko Cultural Association, its staff, or agents responsible for any wrongdoing, claims, demands, or liability.

In the unfortunate event of damages caused by your minor child at KuroNekoCon, you accept financial responsibility and fully indemnify and hold harmless KuroNeko Cultural Association.
