Alpharetta Music City - Music Match Application

1.APPROVAL PROCESS While we will strive to accommodate all applicants, funding and the number of applicants will determine the approval process. Things that can assist in the selection process are, venue size, nights selected, funds available etc. 2.SET-UP AND BREAKDOWN will be the responsibility of the business. Each business is the main contact for the band during the performance and will be responsible to assist in any set up or breakdown needs. Start time and end time will be dependent on location and Musician/Band. 3. ONLY SCHEDULED MUSICIANS will be allowed to perform on event date/time. 1 A-Frame will be placed outside of business advertising the hosted event by the City of Alpharetta. 4.LIABILITY WAIVER AND RELEASE: I, hereby assume all responsibility for, and risks and hazards of, participation in the Alpharetta Music City Music Match planned by myself or my business. In considerations of the City of Alpharetta providing permission to use the space requested, I, and all members of my group, do hereby release the City of Alpharetta, Fulton County Arts & Culture, RCS Productions and hosted musicians/bands, including all officials, officers, employees, sponsors, organizers, supervisors, volunteers, participants, and all other agents, of any and all claims, demands, rights, and causes of action of whatever kind and nature, arising from and by reason of, and all known and unknown, foreseen and unforeseen, bodily and personal injuries, damage to property, and the consequences thereof, resulting from participation in the Alpharetta Music City Music Match planned in the City of Alpharetta.
