This application if for the May 15th ART CRUSH event held on Trade & 6th Street (500-700 blocks) in the Arts District of Winston-Salem in North Carolina.

BEFORE YOU APPLY, PLEASE READ CAREFULLY
Please review all of the information in this application before submitting. If you are selected to participate in this event, you will be charged a $25 booth fee ***Unless you have been selected for an emerging/need-based scholarship.
This application is for May ART CRUSH only. If you’d like to participate in other ART CRUSH months, please complete the separate form for the desired event.
EVENT DETAILS
- Event time: 7–10 PM
- You must stay for the full event. Please, do not pack-up early.
- With load-in and break-down, plan to be there from 6 PM -10:30 PM.
- Vendors will have 1 hour to set up.
- All booths must be fully set up by 7 PM
SELECTION PROCESS
- 40 vendors will be selected per event.
- Vendors will be a mix of artists, creative projects, and creatively driven non-profits.
- If you are selected, you will receive an acceptance email on Monday, April 6th.
- There is a non-refundable $25 fee to participate if selected.
- There are 5 need-based scholarships available per ART CRUSH even.
- Please select the scholarship consideration at the bottom of the form.
- Vendor selections are based on specific criteria to create a well-rounded event, they do not reflect, in any way, your value or talent as an artist; ADWS is an equal-opportunity organization.
BOOTH INFORMATION
- A 6-foot table will be provided for artists.
- Non-profits will need to bring their own tables.
- Arists will be required to pickup their table (exceptions apply)
- Tables will be stacked outside of D.O.S.E on Trade Street.
- If you need assistance with a table, please select this option at the bottom of the form.
- Bringing vendor tents and extra tables are acceptable
SALES & RESPONSIBILITIES
- Vendors are responsible for collecting your own payments.
- Vendors are responsible for charging and reporting sales tax (if required).
ADDITIONAL NOTES
- Participants under 18 must have a legal guardian present if selected.
- Vendor demonstrations are welcomed. Please do not use open flames.
- Please read through the logistics email before you arrive.
- Our event map is not to scale.
- You cannot park on Trade or 6th. Please read our logistics where parking options are listed.
- Chairs, lights, and electricity are not provided. Please, plan ahead.
