About the application

For organizations that offer services such as health screenings, dental exams, blood testing, and other routine assessments that help with early identification of health concerns.  Vendors provide attendees with services free of charge. If your organization is interested in participating as a vendor, complete the application below.   

 

Please note that a valid Certificate of Insurance will be required upon approval.

 

Vendors will need to be set up no later than 10:00 am and stay till 1:00 pm.
