- Do you agree to the City of Gardena Property License Agreement for Vendors
- I understand that a Certificate of Insurance is required to participate in this event. Failure to submit after approval may result in a denial to participate.
- If you have a Certificate of Insurance with the necessary language you can upload now. The Certificate must include “The City of Gardena and its appointed and elected officers, employees, agents, and volunteers are added as additional insured”. The additional insured should include “The City of Gardena, 1700 W. 162nd Street, Gardena, CA 90247”.A sample of the certificate of insurance is included in the "Related Files" section . It outlines the minimum required coverage amounts.
