ALL VENDORS ARE REQUIRED TO PROVIDE:
- Completed Vendor Application.
- Signed and Witnessed Vendor Agreement
- Signed Rules & Regulations
- Signed Waiver of Liability and Indemnification for each person who will work the event
- Signed and Notarized Anti Human Trafficking Affidavit
- Certificate of Insurance (COI) naming Boynton Beach CRA as additionally insured.
- A photo(s) of your products and/or vendor display.
- Valid City of Boynton Beach Business Tax Certificate.
- $100 in gift cards/certificates to your business or if unavailable a $100 VISA or MasterCard gift card by December 1st.

- Upload the COI naming the Boynton Beach CRA as additionally insured here.
- Upload a Valid City of Boynton Beach Business Tax Certificate.
