This scramble style tournament is open to all skill levels. Whether you are putting together a foursome or signing up as an individual player, everyone is welcome to join the fun. Registration includes 18 holes of golf, contests, prizes, and a day filled with camaraderie and community spirit.

2026 JIM BLACKSHEAR MEMORIAL GOLF OUTING
PARTICIPANT WAIVER, RELEASE OF LIABILITY, AND HOLD HARMLESS AGREEMENT

1. ASSUMPTION OF RISK
I, the undersigned, hereby acknowledge that participation in the Jim Blackshear Memorial Golf Outing involves certain inherent risks, including but not limited to injury, illness, property damage, or death. I certify that I am medically able, physically fit, and properly trained to participate in this event. I understand that I am solely responsible for my own safety and well-being while participating in or attending any activities related to this event.
I further acknowledge that this event may involve a test of a person’s physical and mental limits and that risks may include, but are not limited to, those caused by terrain, facilities, temperature, weather, equipment, vehicular traffic, hydration, and the actions or omissions of other participants, volunteers, and event staff.

2. RELEASE AND WAIVER OF LIABILITY
In consideration of being permitted to participate in this event, I hereby, for myself, my executors, administrators, heirs, next of kin, successors, and assigns:

(A) WAIVE, RELEASE, AND DISCHARGE the following entities and persons from any and all liability, including but not limited to liability arising from the negligence or fault of those released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me, including my travel to and from this activity:
•Citrus County Building Alliance (CCBA)
•Citrus County Chamber of Commerce
•Boys & Girls Clubs of Citrus County
•Twisted Oaks Golf Course

and all of their respective directors, officers, employees, volunteers, representatives, agents, sponsors, and assigns.

(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons listed above from any and all liabilities or claims made as a result of my participation, whether caused by negligence or otherwise.

3. HOLD HARMLESS AGREEMENT
The undersigned agrees, on behalf of themselves and any guests or associates participating under their name, that no claim in tort or other law will be made against the Citrus County Building Alliance, the Citrus County Chamber of Commerce, the Boys & Girls Clubs of Citrus County, the Twisted Oaks Golf Course, or any associated organization or individual for any act or omission—whether intentional or negligent—occurring before, during, or after the event.
Furthermore, the undersigned agrees to indemnify and hold harmless all aforementioned entities and persons from any act committed by themselves or their associates that may cause harm or damage to another’s person or property.

4. MEDICAL CONSENT AND MEDIA RELEASE
I consent to receive medical treatment deemed advisable in the event of injury, accident, or illness during this event. I understand that photographs, videos, or other recordings may be taken during the event, and I consent to the use of my likeness for legitimate promotional or archival purposes by the event organizers, sponsors, or partners.

5. GOVERNING LAW AND ACKNOWLEDGMENT
This Waiver and Release shall be construed broadly to provide a release and waiver to the maximum extent permissible under Florida law.

I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I UNDERSTAND THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT IN WHICH I AM GIVING UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND I SIGN IT FREELY AND VOLUNTARILY.
