This Release and Waiver of Liability (the “Release”) is executed on this ___ day of __________, 2025, by [Volunteer Full Name], residing at [Volunteer Address] (the “Volunteer”), in favor of Project PRIDE SRQ (PPSRQ), a nonprofit corporation organized and existing under the laws of the State of Florida, and its directors, officers, employees, agents, successors, and assigns.
1. Volunteer Role
The Volunteer desires to provide volunteer services for PPSRQ and engage in activities related to serving as a volunteer. The Volunteer understands that their relationship with PPSRQ is limited to a volunteer position and that:
No compensation is expected for services provided.
PPSRQ will not provide benefits typically associated with employment.
The Volunteer is responsible for their own insurance coverage in the event of personal injury or illness resulting from their volunteer services.
2. Waiver and Release
The Volunteer releases and forever discharges PPSRQ and its successors and assigns from any and all liability, claims, and demands of any kind, whether in law or equity, that arise or may arise in the future from the services provided to PPSRQ. This includes, but is not limited to, claims for bodily injury, personal injury, illness, death, or property damage resulting from volunteer activities.
3. Insurance
The Volunteer understands that PPSRQ does not assume responsibility for or provide financial or other assistance, including but not limited to medical, health, or disability benefits or insurance. The Volunteer expressly waives any claim for such compensation or liability beyond what may be freely offered by PPSRQ in the event of injury or medical expenses.
4. Medical Treatment
The Volunteer releases and forever discharges PPSRQ from any claim that may arise from any first-aid treatment or other medical services rendered in connection with an emergency during their time as a volunteer.
5. Assumption of Risk
The Volunteer understands that the services provided may involve activities that could be hazardous, including but not limited to lifting, bending, carrying equipment, standing, and walking. The Volunteer knowingly and freely assumes all risks of injury or harm associated with these activities and releases PPSRQ from all liability.
6. Photographic Release
The Volunteer grants and conveys to PPSRQ all rights, title, and interest in any and all photographs, images, video, or audio recordings of the Volunteer or their likeness or voice made by PPSRQ in connection with volunteer services.
7. Governing Law and Severability
This Release shall be governed by and interpreted in accordance with the laws of the State of Florida. If any provision of this Release is found to be unenforceable or invalid, that provision shall be severed, and the remainder of the Release shall remain in full force and effect.
