County Health Permit (Special Events)
Certificate of insurance that includes BOTH an Additional Insured endorsement AND Primary/Non-contributory endorsement
PREPARED FOOD VENDORS MUST ALSO SUBMIT:
CA Sellers Permit (where applicable)
Signed City of Vacaville high-risk vendor waiver / agreement to provide insurance.
